Does anyone have a cheat sheet or other form/tool that can be used to record and analyze a person’s sleep-wake history? I am looking for something I can use when investigating for fatigue as part of our safety incident analysis process. I am also hoping the tool can be used as part of our fatigue management program’s annual effectiveness “check-up”. We will be asking a few safety critical employees to record their sleep-wake patterns in a diary and then taking that information and putting it into a sleep-wake history summary for analysis. Anything you can share would be appreciated….thanks for saving me some time by not having to reinvent the wheel!

Hi EB, in response to your question, I just spoke with one of our founding members, Clinton Marquardt. He agreed to share his Sleep-Wake History Analysis Tool in a blog post. Here is the link to the post where you can download the tool: https://fatiguemanagersnetwork.org/sleep-wake-history-analysis-tool/
